Why Does My Heel Hurt First Thing in the Morning?


Understanding Plantar Fasciitis

Have you ever stepped out of bed in the morning only to feel a sharp pain in the bottom of your heel?

Many people describe it as feeling like they're stepping on a Lego, a nail, or even a bruise. The strange part? After walking for a few minutes, it often starts to feel a little better.

If that sounds familiar, you may be experiencing plantar fasciitis, one of the most common causes of heel pain.


What Is Plantar Fasciitis?

The plantar fascia is a strong band of connective tissue that runs along the bottom of your foot from your heel bone to the base of your toes.

Think of it like the bowstring of your foot. It helps support your arch and stores energy every time you walk, run, or stand.

When this tissue is exposed to more stress than it can comfortably handle over time, it can become irritated and painful.

Although many people still use the term plantar fasciitis, researchers now know that long-standing cases often involve changes in the tissue rather than ongoing inflammation alone. That's one reason treatment often focuses on improving tissue health and gradually restoring function.


Why Does It Hurt More in the Morning?

This is one of the questions I hear most often.

Overnight, while you're sleeping, your foot naturally rests in a slightly pointed position. During those hours, the plantar fascia can tighten slightly.

When you take your first few steps in the morning, that tissue is suddenly stretched under your body weight. Those first steps can feel quite painful until the tissue begins to warm up and move more comfortably.

This "first-step pain" is one of the classic features of plantar fasciitis.


What Causes Plantar Fasciitis?

There isn't usually one single cause. More often, it's the result of several factors working together.

Common contributors include:

  • Standing for long periods
  • Walking on hard surfaces
  • Tight calf muscles
  • Limited ankle mobility
  • Sudden increases in walking or exercise
  • Running
  • Wearing unsupportive footwear
  • Higher body weight
  • Occupations that require prolonged standing

Even small changes in activity can sometimes overload the plantar fascia if your body isn't used to them.


Common Symptoms

You may notice:

  • Pain on the bottom of the heel
  • Pain that's worse with the first few steps in the morning
  • Pain after sitting for a while
  • Tenderness near the inside of the heel
  • Discomfort after long periods of standing
  • Tightness in the arch or calf

Heel pain isn't always plantar fasciitis, so it's important to have persistent symptoms assessed if you're unsure of the cause.


How Can Massage Therapy Help?

Massage therapy doesn't "fix" plantar fasciitis overnight, but it can play an important role as part of a comprehensive treatment plan.

Treatment may include:

  • Reducing tension in the calf muscles
  • Improving ankle mobility
  • Addressing muscles and connective tissue throughout the lower leg and foot
  • Improving overall movement patterns
  • Providing education on activity modification and home care

Because every person is different, treatment is individualized based on your assessment.


Where Does Shockwave Therapy Fit?

For people with persistent plantar heel pain that hasn't improved with time and conservative care, Shockwave Therapy may be an option.

What Can You Do at Home?

Simple self-care strategies may help manage symptoms:

✔ Wear supportive footwear, especially on hard floors.

✔ Avoid walking barefoot if it aggravates your pain.

✔ Stretch your calves regularly.

✔ Gently mobilize your foot before getting out of bed.

✔ Gradually increase activity rather than doing too much too soon.

Some people also find that heat before activity helps the tissues feel less stiff.


When Should You Seek Medical Assessment?

Heel pain isn't always caused by plantar fasciitis.

You should seek medical assessment if:

  • You cannot bear weight on your foot.
  • Your pain followed a significant injury.
  • You have marked swelling, redness, or fever.
  • You experience numbness or tingling.
  • Your symptoms are worsening or not improving despite appropriate care.
